Celebrate your dedication to The Children's Hospital of Philadelphia in your daily travels. We recently arranged for the
creation of specialty CHOP license plates exclusively for residents of the Commonwealth of Pennsylvania. And every plate helps.
With each purchase, a portion of the proceeds will benefit vital programs in the communities we serve.
How much does a plate cost? How much goes to CHOP?
The plate costs $45. All of the proceeds go to programs that CHOP delivers in surrounding communities through the CHOP
Cares Community Fund. You can even make an additional contribution above $45 - all the extra goes to CHOP.
How do I purchase a plate?

2. Complete the required information
3. Send the form with a check, made payable to the CHOP Foundation, to the address listed below:

When will I receive my plate?
Please expect to receive your CHOP plate 8-12 weeks after submitting your
application. If you have any questions regarding the status of your application, call the Foundation’s main number at 267-426-6500.

If you are not interested in purchasing a CHOP specialty license plate but would still like to support the CHOP Cares Community Fund,
you may do so by making your gift online today. By investing in the CHOP Cares Community Fund, your contribution will
